Your Right to a Good Faith Estimate
Under federal law, you have the right to receive a Good Faith Estimate explaining how much your medical care will cost if you are uninsured or choose to self-pay for services.
A Good Faith Estimate provides an expected cost for items and services that are reasonably anticipated for your care. This estimate is provided before your visit or upon request.
Who This Applies To
This notice applies to:
Patients who do not have insurance
Patients who choose not to use insurance for a service
If you are using insurance, your plan determines coverage and cost-sharing, and this notice does not apply.
What Is Included in a Good Faith Estimate
Your Good Faith Estimate will include:

Please note that your final bill may differ if additional services are medically necessary during your visit.
Billing Disputes
If you receive a bill that is $400 or more higher than your Good Faith Estimate, you have the right to dispute the bill through the U.S. Department of Health and Human Services.
