This Notice describes how medical information about you may be used and disclosed and how you can access this information. Please review it carefully.

Our Responsibilities: Aliso Dermatology is required by law to maintain the privacy and security of your protected health information ("PHI"), provide you with this Notice of Privacy Practices, follow the privacy practices described in this Notice, and notify you following a breach of unsecured PHI as required by law. This Notice applies to all records of your care maintained by Aliso Dermatology.

How We May Use and Disclose Health Information Without Authorization: We may use and disclose your PHI without your written authorization as permitted or required by law. The categories below describe the types of uses and disclosures that may occur, though not every use or disclosure is listed.

For Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services, including disclosures to physicians, nurses, medical assistants, technicians, laboratories, pharmacies, and other healthcare providers involved in your care or to whom you are referred.

For Payment: We may use and disclose your PHI to bill and collect payment for services provided to you, including submitting claims to insurance companies, determining eligibility or coverage, obtaining prior authorizations, and responding to payer inquiries.

For Health Care Operations: We may use and disclose your PHI for healthcare operations such as quality assessment and improvement activities, chart reviews, credentialing, training, licensing, auditing, compliance activities, and general business management of the practice.

As Required by Law: We may disclose your PHI when required to do so by federal, state, or local law, including disclosures pursuant to court orders, subpoenas, law enforcement requests, public oversight activities, or government investigations.

Public Health Activities: We may disclose your PHI for public health purposes, including preventing or controlling disease, injury, or disability; reporting adverse reactions to medications or defective medical devices; or reporting suspected abuse, neglect, or domestic violence.

Research: We may use or disclose your PHI for research purposes only as permitted by law, including with your written authorization, with approval by an Institutional Review Board (IRB), pursuant to a waiver or alteration of authorization, or when information has been appropriately de-identified or shared as a limited data set.

Sale of PHI and Fundraising: Aliso Dermatology does not sell protected health information. We will not use your PHI for fundraising purposes without providing you the opportunity to opt out as required by law.

Workers' Compensation: We may disclose your PHI as authorized by and to the extent necessary to comply with workers' compensation laws or similar programs that provide benefits for work-related injuries or illnesses.

Breach Notification: If we discover a breach of unsecured protected health information that compromises the privacy or security of your information, we will notify you in writing as required by applicable law.

Appointment Reminders and Health-Related Communications: We may use and disclose your PHI to contact you with appointment reminders, information about treatment alternatives, or other health-related services, and may communicate via phone, voicemail, text message, email, or secure electronic messaging platforms.

Individuals Involved in Your Care: Unless you object, we may disclose relevant PHI to family members, friends, or others you identify who are involved in your healthcare or payment for care. If you are unable to agree or object, we may use professional judgment to determine whether disclosure is in your best interest and disclose only the minimum necessary information.

Your Rights Regarding Your Health Information

Right to Request Restrictions: You have the right to request restrictions on certain uses and disclosures of your PHI for treatment, payment, or healthcare operations. We are not required to agree to most restriction requests. However, if you pay for a service in full out-of-pocket, you may request that information related to that service not be disclosed to your health plan, unless disclosure is otherwise required by law.

Right to Inspect and Obtain Copies: You have the right to inspect and obtain copies of your PHI that are used to make decisions about your care, including electronic copies when available. Requests must be submitted in writing. Reasonable fees may apply for copying, mailing, or administrative costs as permitted by law.

Right to Request Amendments: You have the right to request an amendment to your PHI if you believe the information is incorrect or incomplete. Requests must be made in writing and include a reason supporting the request. We may deny your request if the information was not created by us, is not part of the designated record set, would not be available for inspection, or is accurate and complete.

Right to an Accounting of Disclosures: You have the right to request an accounting of certain disclosures of your PHI made by Aliso Dermatology. Requests must be submitted in writing and may cover a period of up to six (6) years. The first request within a 12-month period is provided at no charge; additional requests may incur a reasonable fee.

Right to Request Confidential Communications: You have the right to request that we communicate with you about your health information in a specific way or at a specific location to protect your privacy. Requests must be made in writing, and reasonable requests will be accommodated.

Right to Receive a Paper Copy of This Notice: You have the right to receive a paper copy of this Notice of Privacy Practices at any time, even if you have previously agreed to receive it electronically.

Right to Be Notified of a Breach: You have the right to be notified if a breach of your unsecured PHI occurs that compromises the privacy or security of your information.

Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with Aliso Dermatology or with the U.S. Department of Health and Human Services. Filing a complaint will not result in retaliation or affect the care you receive.

Changes to This Notice: Aliso Dermatology reserves the right to change this Notice at any time. Any changes will apply to all PHI we maintain, and the most current version will be available on our website and upon request.
